Partner Order Request

After you have secured an order from a customer, you will need to fill out the Affiliate Partner Order Confirmation Form below.  This will place the order for a closed sale, and put the customer into the Billing System.  

It will charge the customer for the monthly recurring fee immediately, and then the deposit or whole set up fee will be charged in a separate charge. 

Please indicate the amount you charged to the customer, and whether it is a 50% deposit or whole set up fee charge.
